Informed Consent for Coaching

Please read this carefully before beginning coaching work. This form explains what coaching with Nkechi Diallo, MFA, SXI, ABS involves, what it does not involve, and what you can expect from the process. By signing below, you confirm that you understand and agree to these terms.

What Coaching Is

Coaching with Bandwidth is a collaborative, goal-oriented process focused on helping you clarify what you want, identify what's in the way, and build the capacity to realize your goals. Sessions may include conversation, reflection, education, and between-session homework practices or exercises. Coaching is future-focused and action-oriented.

What Coaching Is Not

Coaching is not therapy, counseling, psychotherapy, or medical treatment. It does not diagnose, treat, or cure any mental health condition, and it is not a substitute for care from a licensed therapist, physician, or other qualified healthcare provider. If you are experiencing a mental health crisis, thoughts of self-harm, or medical symptoms that require clinical support, coaching is not the appropriate level of care to start with, and I can help you connect with appropriate resources.

Scope of Practice

As a Certified Sex Coach (Sexology Institute) and Certified Sexologist (American Board of Sexology), my work is educational and coaching-based. I do not provide medical advice, prescribe treatment, or engage in any form of sexual contact with clients. All coaching is conducted through structured online sessions and is strictly professional in nature.

Confidentiality

What you share in coaching is held in confidence. I will not disclose your information to anyone without your written permission, except in the following circumstances:

If I receive a subpoena to release information

If this unlikely situation arises, I will make you aware before taking action.

Your Rights as a Client

You have the right to ask questions about the coaching process at any time
You have the right to decline any homework exercise, practice, or line of discussion without explanation
You have the right to pause or end coaching at any time, for any reason
You have the right to request a referral to another professional at any point

Boundaries & Professional Conduct

Coaching with Bandwidth is professional and conducted through structured, online sessions. It does not include dating, in-person or virtual sexual contact, romantic involvement with your coach, photo or video content, or any OnlyFans-style content. I reserve the right to end any engagement, without refund of remaining sessions, if these boundaries are not respected.

Technology & Communication

Sessions and checkins may be conducted via video/audio on WhatsApp or phone, Zoom, Signal, email, or other mutually-agreed-upon platforms. While I take reasonable steps to protect your privacy, no digital platform can guarantee absolute security. No recording of sessions is allowed by either party, coach or client, and your all information is stored with discretion. By consenting, you acknowledge & agree to this.

Fees & Payment

After your application has been submitted, if accepted as a client, you'll be contacted via email to set up a 15min discovery call at no charge, to answer any questions you may have and schedule your Assessment. I can also assist you in choosing the best package to fit your needs.

Current Packages offered are:

Personal Assessment: Intake, Case Review, & One 50-min Session with one follow-up email with recommendations for a Plan of Care: $250, fully credited toward any coaching package.
Your 30-Day Journey: Intake, Case Review, Four 50-min Sessions over 4 weeks, Weekly Homework (activities and exercises), and unlimited email checkins: $2000 ($1750 if Assessment is complete)
The 60-Day Intensive: This is the Deep Dive! Intake, Case Review, Eight 50-min sessions over 8 weeks, Weekly Homework (activities & exercises), access to unlimited text, voicenote, and email checkins between sessions: $5000 ($4750 if Assessment is complete).

Cancelation & Rescheduling

Payment is due at time of booking. I will do everything I can to accommodate your scheduling/rescheduling needs, questions, discussion, and check-ins throughout the duration of your package. 24hour notice before rescheduling or cancelation is appreciated. Late arrivals will end at our scheduled time. If rescheduling becomes frequent, we'll discuss whether the current cadence still supports your goals and adjust accordingly.
